Identity & Residency

  • Photo ID
  • Social Security numbers for all applicants
  • Proof of state residency (utility bill or lease)
  • Immigration documents if applicable

Income

  • Pay stubs from the last 30 days
  • Tax return from the most recent year
  • Benefit award letters (Social Security, disability)
  • Self-employment income records

Household

  • Birth certificates for children

Tips
