Identity Documents
For each adult household member (18+): government-issued photo ID (driver's license, state ID, or passport). For children: birth certificates or hospital birth records.
Household Composition
Social Security numbers or cards for all household members; proof of citizenship or immigration status for non-citizens; documentation of any household members with disabilities (if claiming a disability preference).
Income Documentation
For all household members with income: recent pay stubs (last 30–60 days); Social Security/SSI award letters; pension statements; child support received (award order or payment records); any other regular income sources. For zero income: signed statement.
Housing History
Current landlord name and contact information; prior landlord information for the past 3–5 years; any explanation for gaps in rental history or prior evictions (PHAs conduct background checks).
