Identity
Government-issued photo ID for the applicant. For children: birth certificate. For newborns: hospital birth record or certificate of live birth.
Citizenship and Immigration Status
U.S. citizens: birth certificate or U.S. passport. Non-citizens: green card, visa, refugee documentation, or other immigration status documentation. Mixed-status households: only documents for non-citizen members are needed; U.S.-born children don't need immigration documentation.
Income Documentation
Pay stubs from last 30 days for all employed household members; Social Security/SSI award letters; pension or retirement income statements; child support received; unemployment determination; self-employment: most recent tax return; zero income: signed self-certification.
Residency
Proof of state residency: lease, utility bill, bank statement, or any government mail at your address. You must reside in the state where you apply.
