Identity

Government-issued photo ID for the applicant. Social Security numbers or cards for all household members.

Income Documentation

All household members with income: pay stubs from last 30 days; Social Security/SSI award letter; pension statement; unemployment determination; SNAP or TANF award letter (automatically establishes income eligibility in most states); zero income: signed statement.

Utility Information

Most recent utility bill(s) with your account number, service address, and utility company name. For heating oil or propane: your vendor's name and contact information. Bring bills for each utility you want assistance with (gas and electric may be separate).

Residency

Lease agreement, mortgage statement, or utility bill confirming you live at the address where you need assistance. Renters with utilities included in rent generally don't qualify (the landlord pays the utility, not you).
